Allergies and Medical Conditions

Children with medical conditions, mental health needs or allergies are valued as full and participating members of our school community. Forestdale understands the importance of medication being taken as prescribed as well as having information to plan in-school activities so not to exclude a child with an allergy. Allergies and/or medical conditions require  special care and attention.

To do:

• You must complete the ALLERGIES AND MEDICAL CONDITIONS FORM* for your child (if applicable) and send it to the school office. 

• Regardless of whether or not your child has an allergy or medical condition, please complete sections 2a and 2b of the RECEPTION AUTHORISATION FORM* to say that you have read and understood the information on this page.
