Allergen Menu Request
Children with severe or multiple allergies registered for school meals need to have specific menus created tailored to their needs. Before a child can start eating school meals, the catering company AIP must be informed of any allergies.
- Please complete the ALLERGEN MENU REQUEST FORM* (if applicable) and email it to the Quality and Nutrition Manager at AIP firstname.lastname@example.org. Please do not email this form to the school office. Please also include medical documentation of the allergies to ensure that we can assess all of the risks to the child. Meals are on a four week menu cycle and your choices will repeat every four weeks. There is no need to reorder each time.
- Regardless of whether your child has a food allergy or not, please complete sections 3a and 3b of the RECEPTION AUTHORISATION FORM* to say that you have read and understood the information on this page.
